The Fitness for Duty workflow, start to finish.

Watch the walkthrough once, then keep this page open the first few times you run an exam. Nine steps: book the appointment, record the exam, let OSHEN fill the form, review it, sign it, and hand the employer a Work Disposition Report.

Same order as the video. Read it once, then use it as a checklist.

Before you start

Open app.saaralabs.health and sign in under your own name. That matters later: OSHEN only lets you sign a form as the person who is logged in. If your browser shows a different address, stop and ask Corey or Bob for the right link.

Saara or OSHEN? Same software. OSH360 calls it OSHEN; the company that makes it calls it Saara. You may see either name on screen.

Create the appointment

Click the + in the top-right corner, then New Appointment.

  • Existing patient: pick them from the list.
  • New patient: click New Patient, fill in the demographics, click Create Patient. They'll appear in the patient list.

Then set the details: the exam type (for example, Return-to-Work evaluation), date and time, how long you expect it to take, the location where the exam happens, and the provider — the nurse practitioner who will conduct it. Click Create Appointment.

Start the examination

Open Appointments from the left-hand bar and click the patient's appointment. A sidebar opens with everything you entered. If the provider or location has changed, fix it here. Then click Start Examination.

Create the recording

New Recording box

A recording and an examination are almost the same thing: one recording, one patient, one exam. You'll see New Recording with the patient's name. Add a note for yourself if you want, then click Create Recording.

The box that appears is the start of your exam. Be in the exam room with the laptop or tablet, and put the microphone between you and the patient. Click Start Recording and run the exam the way you normally would.

How to talk so OSHEN catches everything: ask your questions in any order you like — there is no script. The one habit that matters is to say your findings out loud as you take them: "blood pressure is 128 over 82," "temperature is 98.4." If the patient's employer, job title, or history isn't already in their profile, just say it during the recording and OSHEN will pick it up.
How long? Real exams run 15 to 30 minutes, sometimes longer. Pay attention to the patient, ask the questions, and let the recording do the rest.

Apply the recording to the Fitness for Duty form

Stop Recording → Apply to a Form

Click Stop Recording. OSHEN transcribes the whole conversation into the transcript panel. When it's done, click Apply to a Form and choose Fit for Duty.

Give it 30 seconds to a minute. OSHEN reads the entire recording plus the patient record, pulls out everything it needs, and fills in every field on the form.

Review, edit, sign & lock the Fitness for Duty form

Click Full Page so you can read it comfortably. Every field is color-coded so you know where the answer came from:

Blue

Came from the recording — what you and the patient said.

Purple

Came from the patient record — demographics and history already on file.

Tan / orange

OSHEN wasn't sure and didn't want to guess. You fill these in.

Go through the entire form. Fix anything wrong by clicking into the field and typing. Anything you didn't discuss will say "not documented in the transcription." Check every date. Check that the right boxes are ticked — for example, "may work with the following limitations."

When it's right: type your name to sign off, then click the button to digitally sign. Only the person logged in can sign, and that's the name that goes on the form.

At the bottom you'll see Download, Save as Draft, Send to Clinician, and Sign & Lock. Use Sign & Lock. A locked form can never be changed — you can add an addendum later, but the original stays as signed. Then click Back.

Generate the Work Disposition Report

From the page you land on after clicking Back, click Generate Work Disposition Report. OSHEN takes the Fitness for Duty form you just verified and condenses it into the Work Disposition Report the employer needs. It takes a few seconds.

Edit the Work Disposition Report

Same drill: Full Page, then read every line. Check or uncheck boxes as needed — the evaluation type (Fitness for Duty / Return to Work), the work status (full duty, modified or light duty with restrictions, no work), and the restrictions themselves, which carry over from the form. The rest of the exam information fills in below.

Heads up: in the video, the return-to-work box came through slightly off and Bob corrected it by hand. This is exactly why you review before you sign.

Date, sign & lock the report

Sign & Apply → Sign & Lock

Enter today's date and the time, then click Sign & Apply. From here the report can be downloaded, saved, or sent to a clinician — usually the supervising physician. Finish with Sign & Lock so the report can't be altered.

Finding it later: go to Patients, open the patient, then Documents. The signed Work Disposition Report lives there in the patient profile — download it any time.

Upload outside documents to the patient record

Patient profile → Upload Document

In the same patient profile, the Upload Document button in the corner is where letters from outside providers, lab results, or any other supporting paperwork go — usually as a PDF. Everything stays attached to the patient.

Quick reference

Print this. Tape it next to the laptop.

  1. Create the appointment — + → New Appointment. New or existing patient.
  2. Start the examination — Appointments → open it → Start Examination.
  3. Create the recording — mic between you and the patient. Say your findings out loud.
  4. Stop Recording → Apply to a Form → Fit for Duty. Wait 30–60 seconds.
  5. Review the FFD. Blue = from recording, purple = from record, tan = you fill it in. Sign → Sign & Lock → Back.
  6. Generate Work Disposition Report.
  7. Review the report. Fix any boxes.
  8. Date, Sign & Apply, then Sign & Lock. Download / save / send to clinician.
  9. Upload outside letters, labs, PDFs — patient profile → Upload Document.
